Accessibility and Inclusion Support provides assistance to students who are the primary carer of a person with a disability, long term illness, or is aged and frail and require reasonable study adjustments and support in relation to their role as a carer.

A carer is defined by the Carer Recognition Act 2010 as an individual who provides personal care, support and assistance to another individual who needs it because that other individual has:

  • a disability; or
  • a medical condition (including a terminal or chronic illness); or
  • a mental illness; or
  • is frail and aged.

An individual is not a carer merely because he, she or they:

  • is the spouse, de facto partner, parent, child or other relative of an individual, or is the guardian of an individual; or
  • lives with an individual who requires care.

If you wish to register for support as a carer please complete this form and provide current documentation that includes:

  • information about the condition of the person for whom you care for;
  • whether their condition is permanent, fluctuating or temporary;
  • a statement describing the care the person receives from you

You can download our Carers Health Practitioner Report. This should be completed by the health practitioner treating the person who receives care.

The service will also accept other documentation such as Centrelink paperwork or a document signed, dated and printed on the health practitioner’s formal letterhead if the information provided covers the questions requested in the template.

Personal or health information provided to Accessibility and Inclusion Support is protected by the Privacy and Personal Information Protection Act 1998 (NSW).

When consulting with Accessibility and Inclusion Support, I acknowledge that:

  • information is accurate and given voluntarily
  • information is collected by Accessibility and Inclusion for the purpose of supporting me in my studies and can be disclosed to relevant CSU staff
  • my personal details may be recorded in a file
  • information will be destroyed in accordance with the General Disposal Authorities as set by State Records NSW
  • information will not be supplied to any person or organisation outside the CSU for any purpose unless authorised by me except where CSU may be legally required to do so
  • if I am co enrolled with a CSU partner educational institution, I give permission for my Accessibility Inclusion Advisor to share information with relevant staff at both institutions

I understand that I can request the Accessibility and Inclusion Support to allow me to view my file and request, in writing, that information be updated if it is incorrect. If I am dissatisfied with the way Accessibility and Inclusion Support has handled my personal information I may apply to have the matter reviewed by lodging a formal application with the CSU Ombudsman.
