A carer is defined by the Carer Recognition Act 2010 as an individual who provides personal care, support and assistance to another individual who needs it because that other individual has:
- a disability; or
- a medical condition (including a terminal or chronic illness); or
- a mental illness; or
- is frail and aged.
An individual is not a carer merely because he, she or they:
- is the spouse, de facto partner, parent, child or other relative of an individual, or is the guardian of an individual; or
- lives with an individual who requires care.
If you wish to register for support as a carer please complete this form and provide current documentation that includes:
- information about the condition of the person for whom you care for;
- whether their condition is permanent, fluctuating or temporary;
- a statement describing the care the person receives from you
You can download our Carers Health Practitioner Report. This should be completed by the health practitioner treating the person who receives care.
The service will also accept other documentation such as Centrelink paperwork or a document signed, dated and printed on the health practitioner’s formal letterhead if the information provided covers the questions requested in the template.
