Sport and Blues Award nomination form

Nominations for 2023 have now closed. Thank you to everyone who nominated!

Awards winners will be announced by our Vice Chancellor on Wednesday 1 November.

Thanks for taking the time to help us celebrate the outstanding achievements of our athletes and sporting community.

Nominations close 5pm, Sunday 22 September 2024.

Before you begin

Check eligibility

  1. For individual awards, must be an active student and proceeding towards an academic award and must have completed at least two semesters of study
  2. Sporting results must have been achieved in the period from October 9, 2023 to October 9, 2024

Prepare supporting information

To assist the judging process, it is essential that you supply as much detail as possible and ensure the accuracy of the information provided.

  • We suggest preparing a draft of your submission offline before entering it into the online form to ensure no information is lost if the form times out.
  • High resolution photo/s of the nominated individual, team or club.

Nominations should be supported by letters of recommendation from faculty, coaches, managers, or other relevant individuals who can provide insights into the nominee's abilities, successes, and contributions. Evidence of awards, team selections, achievements etc must also be attached.

If you are nominating for more than one category, you must complete a separate application form for each.
