English Language Proficiency and Student Declaration Form

English is my primary language and I have satisfactorily completed at least six years of primary and secondary education taught and assessed solely in an English recognised country,* including at least two years between years 7 and 12.
I am an enrolled nurse with current registration with the NMBA - English language requirements will have been demonstrated for enrolled nurse registration at the NMBA.
English is not my primary language and I have achieved the required minimum score in the IELTS (academic module) with a minimum overall score of 7 and a minimum score of 7 in each of the four components (listening, reading, writing and speaking) or equivalent test.

I acknowledge that prior to each professional experience placement, I am required to have a current National Police Check, a current working with Children Check and a current First Aid Certificate or CPR certification from my employer.

I understand that registration as a nurse with the Australian Health Practitioner Regulation Agency (AHPRA) is dependent on successful application to the Nursing and Midwifery Board of Australia (NMBA) following the successful completion of my degree.
I understand I am required to have access to a smart device that is capable of using app-based software systems to undertake this course.
I confirm that I have met the English language requirements required for this course and (if applicable) I consent to Charles Sturt University using the information which I provide to verify my results with IELTS or equivalent English Language Test Provider as advised above.
I declare that the information and supporting documents I have provided in this application are true and correct.
I acknowledge that submitting deliberately false or misleading information may result in the cancellation of my application, withdrawal of any offer made by the University, or possible exclusion from the University, as applicable.
I understand that I registration with the NMBA is dependent on my capacity to meet the NMBA Core Registration Standards.
